General Statement of Duties:
Provides oversight and leadership to the agency’s care coordination program and personnel.
Care Coordination is the activity of facilitating communication and information between inter and intra agency providers and between the client/family and providers as needed. Care coordination activities are provided specifically to clients receiving child and/or adult outpatient therapy and/or medical services. This position oversees the Care Coordination Specialist position. This position requires strong interpersonal skills, ability to communicate effectively with a variety of individuals to facilitate timely care for clients and their families. This position will be working closely with treatment providers within the agency and in the community to provide the best possible care for agency clients. The Coordinator works under the direct supervision of the Director of Behavioral Health Services.
Persons in this class are classified Exempt under the Fair Labor Standards Act.
Major Essential Functions:
Provides leadership to care coordination activities;
Provides supervisor and oversight to the Care Coordinator Specialist position;
Responsible for training and orienting new team members to the agency philosophy of care coordination;
Facilitates and coordinates the treatment needs for clients and their families receiving child and/or adult outpatient services and/or medical services from HRCSB.
Provides excellent customer service to clients, staff, and community stakeholders, displays a positive attitude and projects a welcoming, professional image;
Request medical records or other relevant records from community providers as appropriate to facilitate appropriate care of client in HRCSB services;
Facilitates referrals to community providers and other community partners as appropriate on behalf of clients and primary agency treatment provider;
Facilitates communication between agency providers and community providers as appropriate, as well as, communication between client/family and agency providers.
Assists with connecting clients and families to services within the agency;
Assists with discharge planning for clients;
Makes effective use of available technology, including computers, e-mail, and voice mail in order to enhance customer service;
Maintains effective relationships with co-workers and customers based on courtesy, compassion, and respect;
Performs other duties as assigned, consistent with established policy and procedures;

Exceptional customer services skills and a demonstrated ability to communicate effectively with clients and staff; ability to schedule the work of others enforcing agency procedures in a professional and cooperative manner, good organizational skills; ability to handle multiple priorities; ability to work independently; ability to make decisions and initiate action based on established policies and procedures; ability to understand and follow oral and written directions; excellent communication and interpersonal skills; good judgment, flexibility, tact; dependability; attention to detail and pro-active identification of potential risk; ability to maintain confidentiality; knowledge of confidentiality standards and laws; ability to seek and use supervision appropriately; general knowledge of the behavioral health field and the professional terminology is desired.
Minimum Education and Experience:
Graduation from high school or GED OR any equivalent combination of considerable experience and training which provides the required knowledge, skills, and abilities. Bachelor’s Degree Preferred.

Work Environment: General office environment with normal lighting levels, temperature ranges, air quality, ventilation, and noise levels.
Work Location: 1241 North Main Street
Work Hazards: Some hazard associated with repetitive motion necessary for data entry/keyboarding; minimal hazard associated with human services environment.
Work Schedule: Monday through Friday, regular office hours; additional hours as needed to perform job requirements. Regular attendance is required.
