General Statement of Duties:

Work with DD Case Management Supervisor to complete trainings and other assigned duties to successfully demonstrate KSA’s related to the DDCM position.

This position is classified as non-exempt under the Fair Labor Standards Act.

Major Essential Functions:

Complete Case Management training modules and other trainings as assigned;

Read and demonstrate understanding of DBHDS SC handbook

Complete prescribed hours of shadowing of several DDCM support coordinators for the purpose of achieving competency in Knowledge, Skills and Abilities for DDCCM.

Meet with clients, providers, and community agencies as needed to fulfill KSA requirements.

Gain understanding of the DD CM position.

Reports regularly to supervisor to review progress;

Makes effective use of available technology, including computers, e-mail, and voice mail in order to enhance customer service;

Maintains effective relationships with co-workers and customers based on courtesy, compassion, and respect;

Performs duties as assigned consistent with established operational policies and procedures.

Required Knowledge, Skills, and Abilities:

Good interpersonal skills including the ability to empathize, demonstrate respect, communicate warmly and honestly, and allow a client to assume responsibility for himself/herself; honesty; good judgment.

Additional requirement:

Possession of a current, valid driver’s license and acceptable driving record.

Minimum Education and Experience:

A bachelor’s degree in a qualifying Human Services Field

Work Environment: Community settings, individual’s homes, and office with lighting levels, temperature ranges, air quality, ventilation and noise levels as provided in each setting.

Work Location: Individuals’ homes and community settings.

Work Hazards: Minimal hazards associated with general day-to-day community activities.

Work Schedule: Varies according to job and client needs.
