Support Coordination for those with Developmental Disabilities

Harrisonburg Rockingham CSB offers support coordination for children and adults with an Intellectual Disability, as well as for individuals with other developmental disabilities who also receive a DD waiver. Support coordinators assist clients and their families by linking individuals with a variety of public and private services, including medical, social, vocational, behavioral health, and others services.

Our support coordinators use a person-centered approach to develop an individualized service plan to best meet the needs, and to assist with the goals, of each individual.

Get Connected With Screening For Medicaid Waivers

Medicaid Waiver is a funding source for services designed to allow children and adults with disabilities to live as independently as possible in the community.  For some people, this may mean using the Medicaid Waiver to access services that may include moving into a group home or other supported living situation, or to attend community integrated day support program.  For other people, waiver services could be used to make modifications to their own home for accessibility, access supported employment, or obtain needed assistive technology for communication.  There are several types of waivers in Virginia, with access determined by the individual’s level of need.

For adults and children with developmental disabilities living in Harrisonburg or Rockingham County, HRCSB is the single point of entry to gain access to waiver services.  A developmental disability is a severe, chronic condition attributable to a mental or physical impairment, manifested before the age of 22, which result in substantial limitations in several life areas and is likely to continue.  Some examples of developmental disabilities may include:

  • autism spectrum disorder
  • cerebral palsy
  • hearing loss
  • intellectual disability
  • traumatic brain injury
  • vision impairment
