Permanent Supportive Housing Service Coordination Specialist
HRCSB is seeking a full-time Service Coordination position to join HRCSB’s Permanent Supportive Housing team. The Permanent Supportive Housing (PSH) Service Coordination Specialist provides individualized support services to participants to help them maintain stable housing and improve overall well-being. Working within a Housing First model, this position conducts participant assessments, develops individualized service and housing stabilization plans, and connects individuals to community resources such as mental health care, substance use treatment, medical services, employment support, and legal assistance.
The Service Coordination Specialist assists participants with activities of daily living including budgeting, appointment scheduling, and accessing food and healthcare resources. This role also responds to urgent concerns such as eviction risks or personal crises, provides advocacy, and ensures participants remain connected to necessary services.
The position requires maintaining accurate documentation, tracking participant progress, and collaborating closely with the PSH team and community partners to provide coordinated care. Staff utilize evidence-based practices including Motivational Interviewing, Harm Reduction, and Trauma-Informed Care, and support program operations including participant move-ins and move-outs (must be able to lift up to 25 lbs.). Professional development opportunities include training and certification as a SOAR Specialist.
This position requires strong communication, organization, and problem-solving skills, and a commitment to supporting individuals experiencing homelessness and other barriers to housing stability.
ACT Program Assistant
Come join the Assertive Community Treatment team! HRCSB is looking for a Program Assistant to join the ACT team. The Program Assistant is a key player in supporting the ACT team members and day-to-day program operations. This position will provide office based administrative support to the ACT program. This will include clerical duties, record keeping, insurance authorization, scheduling, data collection and entry, and other duties to support the daily functions of the ACT program. Organization skills, attention to detail, experience with spreadsheet and word processing software, and the ability to work collaboratively with a variety of other roles and agencies are essential to this position. Knowledge of mental health, substance abuse, intellectual disability concepts, and basic understanding of practices such as Housing First, Motivation Interviewing, Harm Reduction and Trauma Informed Care is a plus. This position requires a High School diploma with the equivalent of two years of administrative and/or clerical experience OR any equivalent combination of education and experience which provides the required knowledge, skills and abilities.
Accounting Specialist

ACT Peer Recovery Specialist
HRCSB is seeking a qualified individual who is passionate about recovery and empowering individuals to join our team of Peer Recovery Specialists! The role of a Peer Recovery Specialist (PRS) is to provide non-clinical, person-centered, strength-based, wellness focused, and trauma informed support while helping to ensure the individual’s Recovery, Resiliency, and Wellness Plan aids an individual in living a meaningful and independent life in the community. Peer Recovery Specialists are individuals who identify themselves as having lived experience with mental health and/or substance use disorder, or co-occurring disorder, and are in active long-term recovery. Preferred candidates are those who are insightful about their recovery and articulate their experience well to others. Successful Peer Recovery Specialists are able to offer encouragement and support without judgment. PRS will offer services in individual and group settings at the Agency and in the community. This position is part of our Assertive Community Treatment (ACT) team to provide services to adults with a serious mental illness diagnosis, substance use disorder diagnosis, and/or co-occurring disorder. The PRS meets regularly with the team of HRCSB Peers and Peer Program Coordinator for mutual support and guidance. This position is full time and may include evening and weekend coverage.
Assertive Community Treatment Supervisor
HRCSB is seeking a qualified individual to lead our Assertive Community Treatment (ACT) Team as the Supervisor. This position’s primary responsibility is to create and oversee our ACT team and program, in coordination with the Director of Community Mental Health Services. Our ACT team, in a partnership with our Permanent Supportive Housing program, will provide community based wrap around services to individuals with serious mental illness who are in need of stable housing.
The ideal candidate will possess considerable knowledge of adult and geriatric psychology in mental health and substance use disorders; knowledge of treatment planning for individuals having these problems; skills in assessing psychopathology; and experience crisis intervention. Knowledge and experience with ACT model of treatment is a plus. Supervision experience preferred.
Education preferred is graduation from an accredited college or university with a master’s degree in human services, plus 3 years of experience in a mental health, developmental disorders or substance abuse setting or a related human services field; or any equivalent combination of education and experience which provides the required knowledge, skills and abilities. Professional licensure or Licensed Eligible required.

Adult Mental Health Case Manager
HRCSB is looking for a compassionate community-based case manager to work with adults diagnosed with serious mental illness and co-occurring developmental disabilities or substance use. This position is responsible for accessing, linking to and monitoring services and resources for adults in community recovery services. This position requires extensive knowledge of a wide range of individual service approaches and resources for individuals with serious and persistent mental illness. Such as skills in delivering services and crisis interventions and ability to work cooperatively and effectively with individuals. Graduation from an accredited college or university with a bachelor’s degree is required, additional education and/or experience in a mental health, developmental disabilities, or substance abuse setting is preferred. Community Outreach/Intake Specialist
Full time, salaried position to serve as the primary intake person for Intellectual and Developmental Disability services. Activities include meeting with individuals and families to complete functional assessments, adding individuals to the statewide Developmental Disability Waiting list, planning and/or attending community outreach events, and supporting the ID/DD program’s management of the DD Waiver slot allocation process. Candidates need the ability to track and organize numerous concurrent requests for services, an ability to clearly communicate and explain complex systems and procedures, and a willingness to speak to community groups in various public settings. Requires knowledge of developmental disabilities and documentation procedures; and a working knowledge of client rights and local service systems.
